Online Patient Portal:
We are now using a new electronic medical record and billing system. You will now have the ability to schedule, cancel, and modify appointments online. You can also pay bills online.
NEW PATIENTS: A new patient health history information form/Virtual check in will appear 2 DAYS before your appointment. Please log into the portal in that 48 hours before your appointment to fill out your health history information.
Cancellation Policy: To avoid a fee, notify us at least 24 hours prior to your appointment to cancel or reschedule.
